Navigators

The primary function of a navigator is to connect clients to services through referrals and support.  Navigators also assist with defining goals and moving clients along their individualized journey.  The role of a navigator may be called a number of titles such as social worker, care coordinator, case manager, peer manager, or advocate.  Some navigators are limited with the type of services they are knowledgeable about or support in their programs such as Housing or Benefits Navigators.  We have navigators throughout the region that we hope the following resources will help to support in connecting our community services.
